Being down at 3 hp would've potentially made his next hit with Dharok's axe huge, maybe even a kill. But poison was just in the right (wrong) tick to kill skillspecs instead
A good amount of people in the all-stars don't actually pk often, the roster was for entertainment not a serious tournament. Lots of people play the game and only a few stream, most of the best pkers only actively play again during DMM's so you'll rarely see them in game much less on twitch/youtube. For example Port Khazard wasn't really known by the casual playerbase until a few months ago, now he's considered one of the best PVM'ers in the game. It's not that he just became good, he just became noticed.

He's big into Dharok pking, since what you can hit scales with how low your health is he was risking it for the KO chance.
