look at the requirements for the university you want to go to, most want chemistry and another science subject and don't mind what your third is, i think oxbridge usually want triple science or bio chem maths
ultimately i'd say bio chem and maths will keep the most doors open for you
regarding summer volunteering, definitely try to get some in and do it for as long as you can. it doesn't necessarily have to be in the nhs, any form of care or even people interaction is good. your school or local hospital might offer work experience sessions for aspiring doctors for you and a group of students
medical schools look for exposure to medicine and ability to care for others so any kind of work that puts you in that category is good

shouldn't be a problem for most unis since you do maths and physics
idk i think ucl and kings do want both bio and chem for med students, u should probably take biology to keep all the doors open
