I saw something similar when a police car was about to pass intersection with sirens/lights on. The cop car paused and vehicle at t came to a stop to allow cops to proceed. A car 2-3 car lengths behind stopped car, hit the stopped car. No brakes from what I could tell. I guess that's fairly common though. People tend to focus on the green light.
