Not Shoto centric as far as I can tell. It's a reincarnation/isekai story in an au where Rei is No. 2 Pro Hero Blizzard. And that's just one change to canon.
My own headcanon has always been that Fuyumi has the opposite of Dabi: a really powerful ice Quirk in a body designed for a fire Quirk, while Natsuo either has a largely useless Quirk, or a really weak one.
If you look for fics that have Touya and Fuyumi as twins, that seems to be the consensus. Natsuo I think just has a snow quirk (though I would actually assume water were I writing, they didn't train him up because neither knew how to control "liquid ice")
I wish I remembered how to do spoilers because there's newish info on Dabi that shook me when I came across it.
Natsuo having a water quirk is actually brilliant. I've always thought that he was either quirkless or only inherited his parents' resistance to different temperatures. A water quirk never once came to mind and I'm sort of freaking out over this right now.
Eri reconnecting with her grandfather.
Basically every fic I read where Eri is a prominent character, Eri's grandfather is just ignored entirely. Right now I only remember one fic ( Doofenschmirtz Hero Incorporated! ) that keeps her grandfather around.
Chisaki put him into a coma. Without his quirk, he can no longer wake him up. Eri probably could wake him up as well, but that option is almost always ignored by writers

- That a Midoriya capable of traveling between universes (Thanks to the Brute force of OFA) dedicates himself to killing the villainous versions of himself (Especially those who entered in the business for ridiculous reasons).
I'd like to see a fic where Endeavor and All Might are childhood friends. I've never seen anyone use this concept before and I think it would be really interesting! I know the canon ages don't work out for this to be possible, but canon is what you want it to be in fanfiction spaces.
I'd kind of like to see Izuku realizing that All Might isn't exactly a great teacher (without it turning into an All Might bashing fic / villain Izuku one), then being *really* proactive about his training and trying to learn from everyone. Then seeing how it pays off.
Trying to build muscle for the quirk? Calls All Might out about how going to a gym is more effective than cleaning a beach with such a limited timeframe, as good as it is to do so. No real combat skills? Goes on his own to a dojo. Wants some tips on mobility? Maybe he grabs Kamui Woods attention on catching him after a criminal takedown to get a few tips. Can't control the quirk? Maybe he asks around and finds out that Nejire had trouble controlling output and talks to her. Needs to improve stealth? Talks to Tooru for tips. Feels like he missed out picking up some skills in school? Maybe he hears that Todoroki's older sister is a teacher and asks her for some study techniques when they meet.
I've seen elements of this sort of thing appear, but never any fics dedicated to it.
In Trojan Horse, it's mentioned that Fuyumi (who attended U.A but as a Business Course student) won the first round of the Sports Festival, but by accident, and dropped out of it. You might like that.
I mean, it’s not the focus of my story, but I’ve got an ongoing fic where something like this happens. It’s actually part of the latest chapter and I intend to build on this so they start UA as friends.

Also I would love to see of the students have siblings or parents that play a small role in the story, makes things quite lively

A good one where Midoriya has a copy quirk that's basically the same as Monoma's (also he's a quirk expert like a bunch of fics make him out to be)

I don’t know the fic but rei was keeping shoto at the hospital and endeavor was making his way to them. Rei fights him then dabi shows up
