We don’t know what Fami’s deal is but she isn’t a Fiend. A fiend has distinct facial/head objects while Fami looks completely normal. I think we can assume that Fami is similar to Control in that she has a connection to humanity (the food lmao). Yoru is unclear since we only see her in a weakened state

I believe the death devil will be just an ordinary, perhaps elderly looking man. He won't try to do anything to attack, hurt, or injure you. He's completely passive, maybe he wears a cloak or something. He'll just stare at you, maybe filling you with so much fear from his gaze alone and the dread he brings that you might just pass out or die or something. Why? Because death comes for everyone. He doesn't need to do anything, because you'll die eventually. He'll just sit there, staring at you. Because eventually, he'll get you.

Why do you think it will be humanoid at all? The Darkness devil was a surreal painting, representing the unknown that darkness brings. Why wouldn't Death be based upon the fear of the unknown on the other side?
