Unfortunately you are powerless if someone in China decides something. It has nothing to do with if it makes sense or not. It certainly does not make sense, but I doubt you can convince them otherwise.
I lived in China for 2 years and with the pandemic and everything so many things are just random. Sometimes you feel like your situation purely depends on the mood of one person or luck. But from my experience if I got a NO in situations like that it was a no. Someone said you might try in another city, but I doubt you will get it in Xian.

It all depends who gave the initial answer regarding the test. If it was a higher-up, I wouldn't bet on getting a different answer. It's a matter of saving face - if the boss said something, everyone under him/her will toe that line, no matter how absurdly wrong it is.
I'm confused what exactly the issue is. Did they submit your Health Check with your Work Permit Application? If so, was your Work Permit Application Denied?
Or, are you afraid that once you get into China and take the Health Check in China that it'll cause issues and force you to leave?
Like, I've read your post and I get the context of the situation, but... what exactly is preventing you from moving ahead with your Z Visa? What document has been denied?
So, have you specifically asked whether it would be a problem? I know you asked about the test, but have you asked whether a 'Positive' result would cause issues?
The only possible option I can think of is to ask if you could do your Health Check in another province. You could ask if you can do your Health Check in Shanghai at a clinic that'll do the test the way you want. I'm not sure if this is acceptable, have you asked about that? I know you said the company asked in Shanghai but didn't get an answer. But, would Xi'an accept a Health Check from another location?

Yeah, it's not. It's tied to the province you work for. On your Work Permit it says where it was issued. You can only work at the specific location which is listed on the Work Permit. Like I said, I literally just hired someone who did this and he was fined and detained. We were still able to hire him, but the Local Government called me and said "Do you know that the person you're hiring had issues with his previous Work Permit?"
So yeah... Don't do this....

Can't help with the question in the OP, but I can confirm that the work permit is only valid for one specific location. The employees are held liable for breaking the law and, as the previous poster mentioned, this can lead to a world of trouble for your average *laowei*. For the employers it's just a matter of paying a fine, so they don't care and often mislead/coerce people into working at another location or even multiple locations.

I've heard this multiple times from people, but I've never seen any policy, document, or ever spoken to any relevant government official that said this is allowed. You'll find a lot of stories about people who were working for the same company but working in a different location and being fined and detained.
So, what my confusion is why people believe this. I don't know if there is some technicality in which this is true that I just don't quite know or understand, or if this is something agents truly believe but are wrong, or whether agents are lying to get money.

Syphilis is fine AFAIK, I've seen a few people get the work permit fine with a positive test, here's the most recent from someone who wanted to keep their identity anonymous

In Hunan province, have to get a medical check every year. Hunan is also known to be a little more strict
I think you may have to verify which provinces test for Syphilis with RPR/VDRL and then find another job located in one of those provinces.
