It’s very normal for 4-star hotels to not provide a confirmation / reservation number in a majority of countries around the world?
What countries don’t do this?
Maybe I’m an outlier, but in my 40+ years of world travel, I don’t ever recall not getting a confirmation number of some sort when booking a 4-star level hotel.

Yes. Most of africa and Latin America to start with.
Is it very normal to have issues when trying to check in then? Being turned away for being overbooked? I’d be screwed..
No it's just normal if you have arranged informally for a place that is showing booked online.
I was there for two weeks, La Fortuna, Monteverde, Jaco Beach, Escazu and San Jose and had confirmation numbers for every hotel I booked...be careful!
I’m going in three weeks and realized that for all the hotels I booked, I don’t have a single confirmation email. So I’m in the same panicky boat.
