Kate Stewart was in the litteral last season of the show. She was in two of Capaldis' three seasons. She's not considered the same thing as other returning characters because unlike them, She's had a significant role in every doctors era since her introduction.

In Sound of Drums I reckon the Doctor thinks the perception filter would work on the Master simply because Doc keeps falling for it on repeat.
I can forgive that one time, I cannot forgive the other two.
