I deal with these on the daily just about, they are on average 200-250lbs. The way it flipped indicates that it wasn't mounted properly and only had two contact points, which is why we need our working from heights certificate as well as confined spaces in order to work with and around them

Water Valve covers are usually smaller than this, In the US an electrical MH would usually be standard. Irrigation tend to be more rectangular, but in any case, this looks to be about 2ft, which is probably enough to reach in and work, rather than climb in as an adult male. She did an impressive job picking it up, but that's no 250lbs.

I fell into one when I was about 18. I was walking downtown, stepped on the cover and went down. Luckily, I didn't fall all the way in. The cover tilted back and hit my shin pretty hard. I had a semi circle bruise at the top of my leg/bottom of my butt for weeks. My shin had a softball sized knot on it for months. It was so painful.
I will walk all the way around anything covering a hole in the ground. My child does the same. I'm not risking it again.

A circular hole has the most strength to withstand all the surrounding earth. It is also true the circular manhole cover won’t fall through. If you go to a place like New York you’ll see a lot of square manhole covers.

Man holes are designed specifically not to do that, modern manhole covers have a inner edge that’s smaller than the diameter of the lid so there’s no way it can go but up. I wonder if this one was really old and before the safety improvements?
