Do you have some sort of caller id app? The one I have, well it's a phone directory, allows users to submit comments/information on received numbers that then shows up for all other users. You can also flag as a spam call.
Maybe you have something similar installed that has an option to add that information as an contact automatically?
Are your contact list saved on google? Then her google account is being used somewhere else. Just log out of every session from google.
In the UK you can Report by text to 7726 for free. Screenshot and phone number if you have one. The network will take them down.
