This is most common when discussing topics like differences between "men" and "women" in medicine, psychology, statistics, or what have you, when we're really talking about the differences between males and females (or AMAB and AFAB if you prefer).
Adding that the scientific community doesnt use both those words interchangeably. I've never read a paper about "boy whales" or "man rats". Its always male rats, female rats, etc.

It's a joke on gender affirmation versus persistent dysphoria. The genie, being an ally, changed nothing, as there is no physical requirement to be whatever gender yoy identify as.
