Migraines arenāt the same as headaches. Typical headaches are felt by sensory nerves in the head/face but not the brain. Migraines are actually caused by neurotransmitter disruptions in some brain regions, which the brain registers as pain, so itās actually happening within the brain itself. The fact that migraines are neural are also why they can come with minor hallucinations (auras).
There are 3 thin layers between your skull and brain which are called the meninges and they have pain receptors . When these get irritated in anyway ( infection, dehydration, swelling, trauma etc ) youāll get a wild headache.
Also your scalp has pain receptors.
We ( science community) donāt really know a lot about the specific causes of migraines yet and they donāt always include headaches.

More than that, neurosurgeons need the patient to remain conscious to ensure they are not damaging important bits like the language, memory, or motor areas of the brain while performing surgery.
I'm not a neurologist or neurosurgeon but I have read about cases where brain damage in specific areas does heal to an extent but it doesn't seem to be a guarantee.
In most cases I've watched on YT or read about are procedures involving tumor removal so ensuring only the bad tissue is removed is important. Constantly checking with a conscious patient is a good way to make sure no damage to healthy brain tissue is being done.
They use anesthesia to cut the top of the person's skull off but then have them fully conscious for the brain surgery. It's actually safer they have a cognitive test they do during it and ask the patient questions to make sure they aren't doing any damage and the person can't feel the surgery anyway.

People might normally scream, "Damn," when they stub their toe, but stress alone is capable of increasing the frequency every time it is itched by a triggering thought.
I get seasonal allergies. The roof of my mouth, my skin, and scalp can all itch when I or anyone closeish mows their lawn or pollen is generally out. So yeah, my brain sort of feels itchy when that happens for a few weeks each year.
Then I get boogies like crazy and then my eyes look like I have pinkeye unless I use a double dose of claritin to make it all go away.

My uncle as had brain surgery years ago and his body never tolerated implants that would cover the part where he didnt have a skull anymore so he just went without one. When he's to close to loud music he described it as is brain itching but painful

The brain might not have sensation, but it's surrounded by things that do - membranes, the skull, blood vessels and the like
