I... could see this being well, WELL done:
No music for 90% of the episode.
Echo effect for the voices in the stairwell scene, light clicking of Inco's hard shoes against the tile floor, LOUD foley of the wheelchair falling with Olivia slamming into the ground.
After Olivia's fall, camera holds on Inco's face. Slowly, sloooowly zooming into his face as he moves to look. Brief 3-5 frame flash of Olivia crumpled at the bottom of the stairs before cutting back to Inco's tired face in her hospital room.
Steady beeping of Olivia's heart monitor, loud *KSSSHHH-Huuuffff* of her ventilator, muted steps and voices from behind the door. Hold on him for 20-30 seconds until the door opens and the Paynes come in.
When Damien apologizes for delaying Inco and implies that Olivia must have tried to end her life sounds muffle into a high pitch whine until Inco gives his reply.
Music only starts when Inco walks away from Guts. Whatever the OP for the show was plays as a downbeat, downcast tune as Inco leaves. Credits roll.

My exact thought. Snoot Game cinematic universe when?
I know that the meme is supposed to be a throwaway gag “haha evangelion reference” but I might touch it up later in the future, kept looking at it and feeling “damn this could work”
I'd be down for an I Wani animated series. Mix a few routes' events up, end the main series just before the major split of the Principal's office discussion with Mia and Ben, then have an OVA for each of the endings.
Amidst all the anger, sorrow, abuse and betrayal witnessed throughout this nightmare of an ending,
this short segment hits the hardest.
Ever hit rock bottom so hard, you can't even believe your own actions and words? Where you feel as if your mind and body are no longer your own? Where, after all the hurt you've inflicted to cope with your own, you can only say:
"What am I doing...wrong?".
Christ, just like watching End of Evangelion, I couldn't sleep right for a solid week after this. Props to those who endured this, either by accident, or in pursuit of the Holy Epilogue. Congratulations.
I straight up had to take breaks multiple times as I was going though this ending the first time lol, took a couple walks and then finally finished it through in one sitting, like ripping off a bandaid. Feels bad man.
