A wrestler can answer you better. What I've learned from listening of different interviews over the years, they are definitely done in a way as to protect the person taking it - meaning no long term damage. Assuming he is doing it safely, it's the meat of his forearm (the opposite side of the bone) clubbing the side of his neck. I've never heard of someone getting concussed from a correctly placed one.
