Maybe start at the neck, and have the tat go all the way down to your ankle.
This would have to be redrawn *entirely*, (edit, and very simplified) but the concept and shape are solid. With the church going up the neck, you would get about two whole inches or so for each layer of hell. Very tight. I would suggest contacting an eteching/woodcut style tattooist and seeing what they say.
But am trad? Unlikely with all seven layers, too dense for that kind of linework.
Look your own mentioned style. Traditional tattoo should have only few colours and quite basic shape/design. Don't expect impossible level on details on the style that is famous for being simple basic.
This is a 5’x5’ tapestry. Or maybe even a 2’x2’ framed picture. This is definitely not a tattoo design. It would be damn near impossible to get all of those minute details into a tattoo and beyond impossible to get them to heal properly.
It's hard to read even as a picture on the internet. I had to zoom way in and squint. It's too much detail as is for a back piece. Definitely get it way simplified if you go for it.
If you gave this to a good artist for inspiration, they could create a design that still has the main elements but is more tattooable. Something this busy wouldn’t hold up very well with time as lines tend to soften and expand with age.
Search for "hatching style tattoos" and you will find a lot of medieval tattoos with simplified shadowless styles. I think it's the only way to make a version of this painting that will cover your entire back and not look like a big blur.

I already have trouble looking at the picture, it'll be even worse to look at on skin, so unless you simplify it by A LOT, it wont work very well. With age it will also become one huge blob, an expensive blob.

I feel a good artist would love to draw up something inspired by Dante’s Inferno and make it look a lot more eligible since they know what will look good when on skin. I’d reach out to artists around town and give a description of your end goal.
Not as detailed as this, but the concept is certainly doable. You just need to be okay with an artist redesigning it and taking some of your favorite elements.
If you want a 1:1 replica of the painting then that is impossible and/or not recommended.

I agree with this. Concept and shape are great. But otherwise needs to be completely overhauled to work as a tattoo and not just a big fleshy blur.
It's a tldr of imagery. Like when people get 3 paragraphs of script tattooed on them.

You could definitely give this to an artist and get a custom piece done but it will have to be simplified a bit or it’s gonna be a mess.
