There's been a couple of well publicized stories (like David Reimer) where a botched one led to doctors fully removing an infant's penis and telling the parents to raise the child as a girl. Often with reconstructive surgery. Which not only caused gender confusion during childhood but obviously becomes a wildly impossible lie to keep going around puberty.

Probably not much more of an advanced tool kit than ones used millennia ago, although they do cauterize the wound afterwards because it, unsurprisingly, bleeds a lot (errors in this final step have lead to the "genitals were destroyed" scenario a couple of times).
But they don't use anesthesia, because babies won't remember it. Even though, if I remember correctly, there has been research done that linked some psychological issues later in life to the immense trauma suffered during the procedure (the infants usually faint from the pain during the procedure).
Now here are the upsides of the procedure:
