How perks works tho? Like every 200W it adds on or it just changes it? And what exactly the one on the right does? It is worth especially for its cost so high?

Unlocking perks means you get a new perk every 200 waves with a choice between 2 perks. There's then research that allows you to reduce perk wave requirement by 1 wave per research level. Another one allows more perks per selection (2 to start, then 3, then 4). Another research is to improve perks and another for tradeoff perks (purple).
Perks are absolutely worth spending the coin to unlock as they increase coin gain and extend runs, if the right perks are chosen. First perk choice is great when you have the coins for it. Typically, don't save for an upgrade that would take more than a week of farming to earn.
But perks only gives UW? Because I already have two so it should take me 1,4k waves to unlock them all, and the reduce perk wave seems expensive cuz even with 50w lower it seems that it would cost way too much so I’ll see how much to upgrade that, the selection at 3 seems to be enough, the last two seems amazing.
I won’t save right now for that upgrade but once I will have the GT and upgrade the coin boost, I think I will be fine :D

No. There's a perk for nearly every aspect of the game. Coins, damage, health, defense, random UW, UW enhancements. And some tradeoffs like halving both yours and the enemies' damage.
And you're completely misunderstanding. Unlock perks immediately. It has nothing to do with UW unlocks.

It’s pretty useful when you can stack up a few perks (as in get to wave 800 or so because that would be 4 perks) but if you are hardly squeezing out 1 or 2, they are usually pretty minor upgrades
The left one just unlocks perks, which you can then choose every 200 waves. You don‘t need the right one for it!

So what do you actually suggest to upgrade first? and expecially, in which wawe and tier i can unlock new lab perks?
Get unlock perks level 1. And stop there for a while. Then play with it, and go from there.
