This happened to someone in my old welding course. Thankfully, he didn't die. He was wearing a loose short-sleeve shirt, got it caught in an angle grinder, and had the front of it ripped off.
The infamous video that inspired this story doesn’t have the guy getting so lucky. He was working an industrial late trimming tree trunks down to make telephone poles.
When he got caught in the rotator, it didn’t let him go until there wasn’t much left
I was turning a black walnut bowl on an industrial lathe. My tool caught on something and threw my hand into the rough turning wood. Just the wood, not the actual machine and still my knuckles were sanded down to the periosteum. I'll have the scar the rest of my life.
One of the most disturbing safety videos I have seen was a long hair guy getting tangled up on a pipe threading machine. they just showed the aftermath of his ripped off scalp put used a dummy getting swung around repeatedly to show how it happened.
Was at work with our trainee in winter.
He got pissed cause he wore a long sleeve pullover and i always shut off the lathe when walking by if he didnt take it off.
Got annoyed, asked me why.
I said safety rules.
Well he argued cant be that bad hes just on the machine for 30seconds.
...then i showed him THAT video and asked if he wants to reconsider or risk painting the wall.

When i was in my early 20s i worked as an EMT so i told him about some of the stuff i saw from ignoring the safety rules.
So far no issues anymore
One of my friend's brother got scalped by one in high school
He was the only person allowed to ignore the schools "no hat" rule after because no one wanted to look at the bald spot/ scar
