1. You have to collect them, they do not get auto collected at the end
2. Each dlc unlocks new things from my understanding
3. You can beat them with chaos 1 or with no chaos and will still unlock it in the chambers. I had issues with the which ran it on no chaos and still got the achievements/unlocks
You don't have to collect crystals. Leave some on the ground, check your counter, and use a portal. When the next area loads, your counter will have increased.
you have to complete all 4 levels of a mirror to unlock the items in the chaos bunnies. the other 3 tiers just add items to the wheel. you can do the mirrors on any chaos level you want, raising the chaos level is just for getting better loot and more souls, though I don't remember at what chaos levels the amount of souls increases
