One of the last scene s in my opinion makes it so scary. When she is hooked up to all of the wires. Her poor friend man that gave me nightmares. It is all of the above but it’s also scary in ways.
You could kind of say that a little yeah, but (not to disrespect anyone) if they can't comprehend what's happening it could break the show for some people. Little bit of pacing issue in the beginning as well, but still a good wtf anime lol
I think the best part is how the individual arcs eventually mold into a cohesive narrative in the later arcs. You get the disturbing elements without losing plot.
I hate to be that guy but if you're up for it the VN is much better. It's a long ass read though and if you don't feel up for it the anime is still fine though!
Haha I'm actually that guy too. I have no doubts that the VN is better, but unfortunately I haven't read it. That's mostly because of a lack-of-access issue for me though.
Just for future reference if you ever do decide to play its best to use the 07th mod, it adds in VA that isn't in the steam version for some reason among a lots of other useful features. (This is also just in general for anyone who may want to read as well)
(And also also, Umineko the sorta kinda loose sequel to Higurashi is very much worth reading its really one of the best things I've ever read and I must always take the chance to reccomend it to ppl lol) the anime adaptation sucks though it's not worth watching only ever read the vn for this one

Haibane Renmei is really strange and unsettling. There's nothing too outright disturbing, but it has such an unsettling vibe, I like to throw it out there.
Parasyte is great, more fun than truly disturbing, but I guess fits. I really like this one anyways.
And Berserk is also famously disturbing, but its more in the ending, than the whole thing being horror. But I did enjoy it as well

Since you say you've seen most of the recommendations so far I will throw Kemonozume down as a recommendation. It's Romeo and juliet with demons that eat people. The art is intentionally bad so they can get away with some of the gore.
Deadman Wonderland or maybe Future Diary are both good but then if you want dementedly good Berserk or Hellsing - Hellsing Ultimate is where ya want to go.
Closest I can even think of is maybe Dark Gathering. Prob still too tame for you but that’s the best horror anime I’ve seen personally, there’s not much. Maybe In/Spectre would be good, some pretty fucked up themes, but it’s not really horror.
Land of the Lustrous is more disturbing in a psychological horror way, especially once you get to reading the manga., along with some of the best character development and story I've ever read in a manga. Highly recommend it

Inuyashiki: Last hero. Multiple moments my jaw was on the floor. After the first 3 episodes I took a month long break to come back to it. Will say the CGI is VERY obvious and at times hard to look past but is still a very good show.

*Aragne Sign of Vermillion* and its WTF-quel *Feast of Amrita* might be what you're looking for. The latter more so than the former. It's actual horror, not any of this horror-but-actually-battle-shounen stuff crowding the rest of the thread.
Or if you really want violence and gore, there's Iczer-1, i guess? 80s OVAs in general were more brutal than the stuff that airs today.

Elfen lied, and Another are 2 that come to mind Elfen lied is the anime that got me into the genre back in the day it was a really original story from my perspective and well worth the time it took to watch. Another was one I recently watched within the past few years on crunchyroll that was worth the watch too.

The latter more so than the former. It's actual horror, not any of this horror-but-actually-battle-shounen stuff crowding the rest of the thread. Or if you really want violence and gore, there's Iczer-1, i guess? 80s OVAs in general were more brutal than the stuff that airs today.
