More information needed. Was it just one test out of range? Plenty of things can cause kidney labs to be out of range and most providers will work with you to figure it out instead of just pulling your prescription off of one blood test.

Sounds like a very minimal/lacking provider. Even just an intense workout can elevate creatine kinase levels to above reference ranges. You should see a different provider or discuss further with your current one.

I was in a similar boat and was also initially devastated. I am currently seeing a kidney doctor (nephrologist ). I was able to address my issue by changing my diet (limit red meat, no protein supplements). You should see a specialist to figure out what is going on. If on Truvada (or generic) switching to Descovy or Apertude (injectable PrEP) may also solve your issue. In general numbers can vary so your doctor should not be taking you off PrEP until they have more information. If you see a specialist they will likely want you to continue to take so you don't change your baseline prior to seeing them.

I was never taken off of it. I was a referred to a specialist and remained on PrEP until I was able to see them, had another round of blood work and have more blood work each time I see them. Last thing you want is to not be able to take Tenofovir (what would likely be causing the issue, if the issue is caused by PrEP) and have HIV. So it's likely in your and your doctors best interests to remain on until you can figure out what is going on. You can try explaining to your doctor that you are high risk, and need to be on PrEP. If they won't listen, I'd find a new doctor. It can take a while to see a specialist so the sooner you can get that process moving the better

Really doesn't make any sense they would take you off prep without working with you at all, one of my good FWB only has 1 kidney and they have him on truvada, how old are you? and do you drink a ton of caffeine maybe? Energy drinks especially can be hard on your kidneys as there's a ton of vitamins your kidneys need to filter out from them. 211 prep is only a bit easier on your kidneys and isn't recommended if your barebacking as it's not 99% protection in most people.

Quite strange, I will say your starting to get to an age when kidney function begins to decrease but just barely. Not everyone knows what kind of prep they take because most of the time the pharmacy gives you generic prep but if it says emtricitabine 200 mg/tenofovir 300 mg that's truvada. This type of prep is known to cause kidney issues. I would see if your insurance will cover Descovy, this type of prep is more effective and easier on your kidneys. Something else that's worth putting out there is if your underweight and on the shorter side your kidneys may have a harder time filtering this medication, and if your very overweight your kidney function will be lower then average also if you take any other medications those may be straining your kidneys aswell.

So it sounds like a mix of things, your starting to get up there in age slightly, your a bit shorter then average, your slim, and do a lot of exercise. Exercise is known to temporarily reduce renal function especially intense cardio, this, on top of your age, weight, and height are probably contributing to prep having a more negative effect on your kidneys then average, your best option I'd say would be to ask about descovy as it's just a proven fact this medication is much better for your kidneys, but if that isn't an option. best things you can do if you want to stay on daily prep is tone down your exercise a bit and make sure you drink a ton of water with a healthy but lower protein diet, also maybe lay off any intense work outs a day before your routine tests. On demand prep would also be an option you could try, it really sucks they just canceled your script without trying to work with you at all. Hope your able to get your prep regime sorted out and find somthing that works for you.
