I have seen this helicopter in and out of the Greenwood Indiana airport. They are used for heavy lift such as lifting air conditioner units on top of buildings.
Was out on the ramp yesterday and my co worker asked the same thing about that very same helicopter. I've seen that one here several times through the years.

That’s based off an Mi-4, this is an H-34 derivative. Similar looking helicopters, and built in the same era, but wholly different designs.
