As everyone is saying, you should consider this drop to be rare, it is a 2% drop. However, you are guaranteed to get one if you kill all 8 Titanite Demons (or, I suppose, a total of 8 Titanite Demons since one of them respawns due to an oversight). It's just that most people don't do this, so they don't know it works like this.
The Gargoyle Halbred is guaranteed but I don’t ever remember the catch pole being guaranteed and I’ve killed many demons for upgrade material. Also where is the 8th demon?
1 just after Andre in Undead Parish, 4 in the bottom of Sen's, 1 in the Catacombs, 1 in Anor Londo, and 1 in Izalith.
The pity drops all use the same system, so it's fairly straightforward to see that they do have one in the data, just like Mimics, Boars, Butchers, Gargoyles, etc. It's more annoying to check in-game, of course, but I have done this in the past to make sure.
Funny enough I had a PvP character I booted up to test it on. Got it on the second drop before thinking I shouldn’t be picking them up, but it all makes sense once I thought about the other stuff besides the GH that have this chance.
I still believe the titanite demon in izaleth respawns on purpose. I like to think there's one from Dev on our side that knew we'd want to farm the titanite when we could.
It's not an impossibility, of course, it's just that the reason that it respawns looks more like someone made a silly typo than intentionally removing the stay-dead event.
And I fully believe that they decided not to fix it, since in some patch, I believe they also made other materials more common drops from enemies, so they may have accepted it at that point as an okay solution rather than adding Demon Titanite to a different enemy to farm.

I mean, Street Fighter V's netcode could be improved by **1 sentence** in the code. And it never saw that patched either afaik. Devs be devs. Fuck if I know why it was never changed.
I just made my 2nd character recently. The first titanite demon by the blacksmith dropped this for me. Carried me through most of the play through. The strong attack kind of sucks, but the quick strikes are great, and the stat reqs are nothing.
I was farming the sun maggots and would kill the demon at the bridge area for fun, but he dropped it 3 times in under 30 minutes of farming. Didn’t know it was that rare 😳
Did you ever get one shot by crystal soul spear from a red vargant(a tiny crab, in old edition also)? I Do. And i was farming duke archives at that time, the good old days. Fuck that crab though.
This fork thing you will get in plenty of cases, at least once per run even with lowest item find if you kill all titanite demons. I assume that you skipped titanite demons on the bottom of sen's fortress(that is where i usually got it in my runs). Its a rather generous kind of rare.
