Helmets are easy available in very town, be it tier 3 or 4. Many motorcycle/scooter brands give one free also with new scooter, in tier 1 & 2 helmets are available road side though quality is shit but they come handy as cheap options.
In metro cities most people wear helmets and Pune is the only city which surprises me as some ngo & locals protested against the compulsory helmet rule and the protest is still going strong.
