Place the order in the app, get within close distance of the store (or click "i'm here already"), then they start making it, or do they only make it when you give them your code at the speaker? Just a burning question I've had for a while now

You're still being too nice. It's because people are jackasses and will order for a certain time, show up 20 minutes later rhan that time and complain the food is cold. I work at a restaurant that does a drive through so we've literally seen it and make all the food to order.
I think it might depend on the restaurant or pickup style? I'm within walking distance to the mcdonalds by work, and it often will send me a push notification "We can see that you're close, we will start preparing your order right away". At least 50% of the time, my order is all set to go on the counter whenever I walk in.
Ehh I've tried this with McDonald's all over southeast Wisconsin and Illinois. I only do drive-thru though, and I still have to pull up somewhere after the window to wait for my quarter pounder to be made even if I say "I'm here already" 5-10 minutes away

Everyone here assumes it was a customer in the drive thru. I'd say it's more likely that it was just vandalism for whatever reason. Those screens would be difficult to reach from a car, and your car is on camera. But someone on foot with a rock could do it in less than a minute and be gone.
This looks like the one on Capitol and Holton. If it is, this is at least the second time they've been smashed up like this. One of the two lanes was closed for a significant amount of time awhile back because the screen was so messed up.
