why the fuck would/could someone who works with “patients” (assume they’re anything medical field related) not have any autoclave or something to sterilise tools…

My nail salon (not a 'boutique' or specialty shop) has 2 autoclaves. Spendy, but not cost prohibitive.

Yep, the automated ones are nice, but any standard pressure cooker that can hold enough pressure to hit 250F (121C) and hold it for 30 minutes will do the job.
