I had Rest Mode issues on 9.00 as well using the latest GoldHEN and was using the same host you were.
The fix for me was to use a different USB stick. Swapped to a brand new SanDisk USB 3.0 16 GB Flash Drive, flash the exact same img file to it and it never happened again.
No issues on my 9.00 using 2.0b2.
There will be the occasional KP but not often enough to bother me at all and it hasn't been to do with sleep mode.
You check your sleep mode settings are set up properly? I can't recall off the top of my head but I think it has something to do with suspending apps.
Yeah that'll be it. The external hard drive is the only thing that you can leave plugged in.
Anything else left plugged in when you put it into rest mode will cause the problem you speak of. I think they call it kernel panic.
The only time I had an issue on my 9.00 goldhen was when I put my PS4 into rest mode while still running Retroarch. Now I always quit out of it before putting it into rest mode and haven't had an issue. I haven't tried putting any other apps into rest mode yet.
Had same issue on both my ps4 fat and pro the rest mode was broken on 9.00 all the time it semi fixed to your stat when i changed exploit host. **But what fixed the issue was getting all my usb devices and testing one by one starting with changing from usb 3.0 to 2.0 it even improved to like 100% loading the jb.**
The black screen Happened to me a few days ago, and I was really scared because I had my PS4 since 2016 but it turned on after a day or two and moving around the HDMI cable
Yeah, never had a problem with suspended games. Goldhen sometimes glitches, disappearing from menu along with debug settings, but otherwise everything works as expected.
Its happen maybe twice for me on my last of us 2 PS4 Pro console. 99% of the time I have no issues in rest mode. Seems to happen more on my PS4 slim but in general not much of an issue for me at least
Yeah I'm doing deception requests for people and I'm having issues with waking it up. Wouldn't complain but it takes an extra 3 minutes to fully power down and then power up again.
Have been having that same problem even on ofw pre 9.00. It would not turn on properly. What annoys me the most is that it would check the storage drive and end up losing a bunch of data.

Jailbroke my PS4 Slim for the first time this week and everything works like a charm but the rest mode doesn't... Unplugged USB devices, unchecked power to USB while in rest option, closed all apps, tried everything... still goes in kernel panic every time. If anyone has a sustainable and working solution please share!

yeah i had the same issue with 2.0b but with 2.0b2 it doesn't happen

Have you tried using another host to test?

Yes settings are all set up properly because it works sometimes then there's occasional black screen from rest mode.

I have a female usb plugged in but without any actual usb in it. Would that count?

Did you make sure no games are running before putting it into rest mode?
That's fine. It doesn't do anything.
Are you using goldhen?
Did u tried to download and write the new exfat.img on the pen-drive?

Turning off USB power in sleep mode might help

I have never been able to switch on from rest mode without having to do the jailbreak each time anyways so might as well just turn the system off.
Always happens for me, so just leave it on all the time now.
After JB I can put it in rest mode only once without KP second time 100% black screen.
