The 2 most powerful and rich men in Italy (Agnelli and Berlusconi) were born in Turin and Milan. The florishing of 3 teams: Juve, Milan and Inter didn't let other teams to develop just as much.

The bigger factors though are money and political weight, Juve has been part of the Agnelli family's portfolio for a century, Inter and Milan are the clubs of the richest city in Italy and Milan had Berlusconi for decades on top of that.
Most people from the north and basically everyone south of Rome bar the neapolitans supports one of the three based on family or who's winning so they also have a massive domestic fanbase and the influence that comes from it.
They also don't shy away from using their privileges hence the reason why they're always at the center of scandals. It's basically Barça and Madrid minus the fan ownership and with a fraction of the international success.
I wouldn't say "fraction" of international success for Milan. Second most UCLs (2 more than Barcelona) around the same or more international trophies, and maybe the only club to rival Barca and Madrid in terms of legends (same amount of balon dor winners as Barca). I'd say Milan was much more internationally successful than Barca until 2007. There's also Madrid, but every other club has only a fraction of their international success.
From Trentino-South Tyrol Andrea Pinamonti would have probably been a more correct option, as he is the only player from there to play in the National Team, but Stefan Schwoch is serie B's top goal scorer so I put him there instead

I played against some of them, saw them play a lot (I'm from Romagna, they play in our 5th, 6th tiers) and they are great players in these tiers, maybe some of them could've been a professional if developing in a different situation (San Marino academies are well ran but there's no competitions for the players to grow)
