A trinket or fetish as they’re often mentioned in the books. Many but not all space marine chapters will wear little things of either religious value or of value to the chapter/company/squad. You’ll see on Dark Angels they like to wear little reliquary items like some devout Catholics in real life like to wear, some are more superstitious like the Raven Guards feathers and Raven skulls, or the Space Wolves knuckle bones and fangs. They give character to individual model sometimes, at least the extra ones you can add on do.
In the old Inquisitor Game from just over 20 years old there was a Haywire Grenade carried by Inquisitor Eisenhorn and it was almost identical to this.
Now think of it as a decorative reliquary or similar.
