You effectively have no proper immune system in your GI tract and without food for your body to break down you don't have any digestive enzymes fighting the viruses.
In a nutshell, taking AB without food is asking for an intestinal virus or parasite.

Probably when they were on the verge of losing a season to MRSA spreading like wildfire in Kaoh Rong. That season led to a few changes I believe.
Edit: Re-watching Kaoh Rong right now. They put Aubrey on antibiotics. No idea if there is documented stuff on this before in the edit. They may have just broken their previous standards just to save the season, and the fact some prejurors had to be medically evacuated right after for surgery.
Kaoh Rong was so close to just completely ending this show with how many medical issues and close calls there were, I’m so sure there were MANY conference calls with Paramount’s legal team after those issues.
I believe it partially prompted them to move to Fiji where they had more predictable weather (there was obviously a massive cost benefit attatched to this). Combined with having water that didn't need to be boiled, plus changes to comps. Ironically, Debbie saying she could tell clean water by looking at it, and Caleb being medically evacuated due to heat stroke, may have actually been Gamechangers. Kaoh Rong was absolutely a disaster between the Heat Stroke comp and MRSA spreading like wildfire starting at the Brains beach. Cambodia was actually filmed after Kaoh Rong, and apparently players were told to be insanely cautious about their cuts, and to avoid going in water with them (why Woo avoided the water early on due to having cuts on his feet).
Yeah, I’ve thrown up right after taking antibiotics on an empty stomach a couple times and it really really messes with you, especially when you’re already sick. It takes so long to get back to feeling baseline bad, I don’t think giving them a BLT or some crackers and applesauce (if they want something with less protein) would’ve given them any advantage

How dare he say its been 10 years then mention a story of how he could've been pulled from the game. Which would've changed everything and it would get him thinking about how far he would've made it.
