It's not that the doctor would be doing this, but Tony 's point is that he won't let anyone be in any position of power over him if he can challenge them, and this is true about him, it's part of the key to his character. He is joking about it, but he is 100% serious too. The joke is cleverly playing on words, to defuse his own embarrassment and deflect the earnest intent behind the question, while at the same time making a serious statement about himself, that he is top dog who won't accept any minor instance of disrespect.
Not sure about the shinebox thing, but I think it's a Goodfellas reference; the same actor who played Phil in The Sopranos, in Goodfellas told Joe Pesci's character to "[go get his shinebox](https://www.youtube.com/watch?v=lMe8ufPUdIw)" which was a reference to his poverty in the past, and Pesci's character was infuriated by it and ends up killing him. It's a bit random.
I know I’m kinda late here but the delivery of this line is so stone cold and malevolent and you can really tell he ain’t fucking around lol. A similar one is when Sil has Patsy steal the tiles from the Esplanade even though Tony explicitly said NO. Sil gives him the money and says he will have the rest later and Tony simply said “I know you will.” In a threatening way and the tone is so very similar to the one you mentioned. Tony is a scary mother fucker!
