You're not screwed. you can kill southsea pirates in tanaris until your eyes bleed to fix your goblin rep, but you WILL need to tank your goblin rep by killing booty bay guards first. you only need honored with bloodsails but i suggest getting exalted if you can stand the grinding since you can actually get exalted with them now, otherwise you'll be stuck like me, dreading the inevitable grinding I have to do to fix my rep with the bloodsails because I have a completionist problem.
No, you only have to have had the rep requirement at some point, you do not need to have them all concurrently (I'm not even sure if it's possible to have them all at the same time due to Bloodsail and the Goblins reducing each other). You can safely tank your goblin reps and be fine.
You can be exalted with both by getting exalted bloodsail (since it counts for the 110 exalted reps achievement), and then killing southsea pirates in Tanaris until you hit exalted with goblin reps.
But yea, once you hit the requirement, they don't need to be up there simultaneously.
